rueckenschmerzenOnce in a lifetime nearly every person suffers from pain in the region of lumbar spine, thoracic spine or the shoulder-neck-head region due to cervical spine pain irradiation. Thereby we have to distinguish so called functional musculoskeletal disorders from real degeneration of the ligament (ligamentary), vertebral joints (arthrogen), musculature (myogenic) or the intervertebral disk (discogenic). In case of pressure on the nerve tissue, sensation disorder (sensitive deficit) and paralysis (motor deficit respective paresis) appear in addition to irradiating arm or leg pain.

Nerve pain

nervenschmerzenAt a certain threshold value, pain sensitive nerve ending (nociceptors) in the skin are stimulated and carry this message via electronic impulses through the peripheral nerves to the spinal cord where the incoming signal is connected and transmitted to the brain (cerebrum); only there in the deep brain region – the thalamus – the pain is experienced. In case of direct disorder or damage of the nerve tissue we call the pain neurogenic or neuropathic. We can treat it with anticonvulsants, antidepressants, neurolysis, denervations, TENS machines or electric stimulation of the spinal meninges.

Headache and facial pain

KopfschmerzenThe international classification knows 212 different types of headache. The most common type is the tension headache (dull and pressure-like), migraine (intense throbbing, sometimes accompanied by vision problems and paralysis), spinal headache (cervicogenic respectively vertebrogenic) and facial neuralgia (e.g. trigeminal neuralgia). In order to find the cause it is extremely important to keep a headache diary.

Osteoporosis & bone pain

knochenschmerzenIn case of bone softening (osteoporosis) the balance between the bone formation (osteogenesis) and the bone breakdown (osteolysis) is disturbed; it results in a loss of bone mass. The pre-stage with a reduced bone density is referred to as osteopenia. As the disease is progressing, bone instability and the danger of bone fragility increase, which leads to fractures. From the therapeutic point of view the nutrition advice with regard to boosting bone formation by means of calcium, vitamin D, bisphosphonate and calcitonin plays a decisive role. Also the controlled mobilization with the help of physiotherapy counteracts the bone breakdown.

Joint disorders

gelenkbeschwerdenEvery fourth inhabitant of Germany suffers from osteoarthritis, among those above 65 even every second person. Symptoms are stiffness at rest, mobility degradation and pain at the beginning of a movement. Chronic inflammatory diseases (e.g. rheumatism) or metabolism disorders (e.g. diabetes) favour the occurance of osteoarthritis. The progressive damage (degeneration) of the joint cartilage and impairment of the sliding ability is decisive. As a result painful irritations of tendons and bursa occur additionally.

Muscle pain

MuskelschmerzenThe detection of trigger points painful under pressure is the most important symptom for myofascial pain syndromes (functional unit of muscles, fascia and tendons). The pain occurs during passive muscle stretching and active movement against resistance (muscle dysfunction). The following disease names overlap or are synonyms: soft tissue rheumatism, fibromyalgia, muscular dysbalance and fibrositis. Trigger points infiltration (neural therapy) and administring muscle relaxant medications are successful treatments.

Tumor pain – no fear of opiates

tumorschmerzenBeside the actual treatment of tumor, it is also important to sustain a high quality of life of the patient as part of palliative care as long as possible. Often a quality pain relief can only be achieved with the help of morphine derivatives (opiates respectively opioids). The substances are well-tolerated and have little side-effects; nowadays the medication is available in different forms (pills, capsules, drink liquids, patchs, injections, pump and port systems).

FibromyalgieThe spectrum of psychosomatic diseases has constantly increased in the recent years and fibromyalgia has been also added to the list. The patients suffer from general muscles, tendons and fascias pain in different body regions, though no evidence of inflammation can be collected by means of blood tests or apparative diagnostics. Often patients also complain of painful papules (myogelosis).

Perfusion and pain

durchblutungPerfusion disorders of the lower members affect about 5 to 10 % of the German population. The artery stenosis or even occlusion as a result of arteriosclerosis is called peripheral artery disease (PAD). The distance people can walk without pain is an important indicator for the disease gravity. As patients frequently have to stop for a period of time, in Germany the disease is also called "intermittent claudication".
